Ergonomic Equipment Purchasing Instructions

Employees are responsible for ordering equipment that has been recommended as part of their ergonomic assessment.
For items that require installation contact your local building services or facilities group.


FOX Studio Lot

  1. Forward your report to Cynthia.Diaz@fox.com and include the below information for Cynthia:
    • The type of equipment recommended. (i.e. Haworth Zody Chair)
    • Your department’s billing code.
    • Your building number and office location.
  2. From the time your order is processed, delivery could take 4-8 weeks.
  3. If you have any questions concerning your order, please follow up with Cynthia directly. EHS does not keep track of orders and it is the employee’s responsibility to confirm and keep track of.
  4. For FOX Studio Lot, 1440 Sepulveda and the FOX Plaza, once Building Services receives the equipment from the vendor, they will contact you to setup delivery and installation.
  5. For equipment adjustments or order follow-up questions, contact alexis.guzzard@fox.com.

FOX Television Stations and TVHO:     To purchase recommended equipment, please send one email to your department manager and the HR Business Partner for your location.
